Stuart typically sees patients Tuesday through Thursday. To schedule an appointment, we need to know if you are a New Patient or if you are coming for continuing care.
• • • NEW PATIENTS need to call or email Stuart to schedule an ‘Initial P.T. Evaluation’ appointment. Once the appointment is scheduled, you will receive an email confirmation. Please select ‘Change/Cancel Appointment’ to fill out the form with your personal information. The Initial Evaluation appointment usually lasts more than an hour and possibly as long as an hour and a half.
• • • IF YOU HAVE SEEN STUART BEFORE, but it has been more than three months or if it is a new issue, please call or email Stuart to schedule your appointment. Once the appointment is scheduled, you will receive an email confirmation. Please select ‘Change/Cancel Appointment’ to fill out the form with your personal and insurance information. If you are a current patient please call 503 936 8640 to set up, change or cancel an appointment.

You will receive a reminder email 24 hours before your appointment. We require 24 hour notice if you are not going to make it to your appointment or you will be billed for the time.
WHAT TO BRING
Please bring your prescription or referral to your first appointment. Clothing suitable for physical exam – shorts and t-shirt or tank top. On subsequent appointments, bring clothes you are comfortable exercising in.
